Job summary

Catholic Health Home Care is seeking an experienced Home Health RN to manage comprehensive care plans for patients in home settings. You will collaborate with physicians, therapy teams, social services, and home health aides to support a diverse caseload.

We value hands-on nursing, patient education, and consistent care delivery, with a focus on disease management and health promotion. A NY state RN license, BLS, and reliable transportation are required.

Qualifications

• New York State RN license.
• Minimum of one year of hands-on experience as a Registered Nurse.
• Basic Life Support Certification.
• Valid New York State Driver’s License.

Responsibilities

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for patients.
• Measure vital signs, administer IVs, change dressings, clean wounds as needed.
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with diverse patient populations.
• Work closely with a supportive patient care team including physicians, therapy, social services, pastoral care, home health aides and others for your caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the highest standard.

• 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details

Our Home Care RN’s:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications:

• New York State RN license
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification
• Valid New York State Driver’s License and reliable transportation

Salary Range

USD $100,000.00 - USD $130,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
