Job summary

Baystate Health is looking for a full-time Community Health Worker (CHW) in Springfield, MA. This position involves engaging with patients and their families, conducting home visits, and serving as a vital link between patients and healthcare services.

Qualifications

• Enrolled or willing to enroll in the CHW certification program.
• CHW certification preferred.
• Some exposure to working in a community setting preferred.

Responsibilities

• Develop drive and determination to learn and grow in the CHW role.
• Engage positively with patients, families and the medical team.
• Act as key point person between patients and providers of healthcare and social services.

Skills

Excellent interpersonal, communication and writing skills

Strong time management and organizational abilities

Proficiency with basic computer skills, Microsoft Office and internet research

Knowledge of confidentiality and handling sensitive information

Willingness to work some weekend or evening hours as needed

Education
