Job summary

Qualifications

• 2 years of community-based work experience required.

Responsibilities

• Facilitate telehealth visits between patients and pharmacists.
• Help patients with medication reconciliation and adherence concerns.
• Reinforce pharmacist recommendations with patients.
• Assist with CGM procedures and education.
• Provide diabetes self-management support and health education.
• Document encounters in the EHR and send summaries to providers.
