Responsibilities

• Screen dyads for unmet care needs and psychosocial well-being.
• Guide dyads through advanced care planning with the MACC team.
• Connect dyads to local community services, including respite care.
• Collect medication information, assist reconciliation, and support adherence.
• Work with care team to assess medication side effects and function changes.
