Job summary

Duke University Health System is seeking a Pediatric Transplant & Cellular Therapy Nurse Navigator in Durham, NC. This on-site, Monday–Friday role provides care navigation and coordination for pediatric post-transplant patients within a multidisciplinary team.

The position emphasizes longitudinal follow-up, patient education, and collaboration across clinicians, care coordinators, and insurers to ensure timely access to care and safety. Relocation assistance may be available.

Qualifications

• Bachelor's degree in Nursing required.
• Master's degree in Nursing preferred.
• Current NC Registered Nurse licensure required.
• Minimum of three years of nursing experience required.
• Pediatric nursing experience preferred.
• Experience in nurse navigation and care coordination strongly preferred.
• BCLS certification required.
• Demonstrated ability to communicate effectively.
• Strong clinical assessment, triage, prioritization, and problem-solving skills.

Responsibilities

• Provide individualized care navigation and clinical support for pediatric post-transplant patients and families across the outpatient continuum.
• Serve as a consistent point of contact and primary liaison among patients, families, the multidisciplinary PTCT team, primary care providers, and administrative partners.
• Coordinate longitudinal follow-up care, including clinic visits, laboratory monitoring, imaging, referrals, survivorship needs, and transition points in care.
• Conduct nursing assessments, triage patient and caregiver concerns, and facilitate escalation to the appropriate clinical team member.
• Support patient and family education related to post-transplant expectations, medication adherence, infection prevention, and follow-up schedules.
• Identify and help address barriers to care, including access, scheduling, insurance authorization, transportation, and psychosocial needs.
