Qualifications

• Bachelor's degree in Nursing required; Master's preferred.
• Current or compact RN licensure in North Carolina.
• Minimum of three years of nursing experience.
• Pediatric nursing experience preferred.
• Experience in nurse navigation, care coordination, oncology, hematology, transplant, cellular therapy, or related pediatric care preferred.
• BCLS certification required.
• Strong communication, clinical assessment, triage, prioritization, organization, and problem-solving skills.
• Ability to support longitudinal care across complex systems.

Responsibilities

• Provide individualized care navigation and clinical support for pediatric post-transplant patients and families across the outpatient continuum.
• Serve as a consistent point of contact among patients, families, PTCT team, PCPs, insurance partners, and administrative teams.
• Coordinate longitudinal follow-up care, including visits, labs, imaging, referrals, survivorship, and transitions.
• Conduct nursing assessments, triage concerns, and escalate to the appropriate team member.
• Support patient and family education on post-transplant expectations, medication adherence, infection prevention, and follow-up schedules.
• Identify and address barriers to care, including access, scheduling, insurance, transportation, and psychosocial needs.
• Partner with leaders to improve workflows, patient experience, quality and safety.
• Maintain accurate documentation and promote clear communication among care team and external partners.
