Job summary
Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) to support medically fragile children during transitions to home care. The Transition Nurse role focuses on safe discharges, preventing readmissions, and ensuring continuity of care until permanent staffing is in place.
Preferred candidates have trach, vent, G-tube, and emergency management experience and are comfortable with high-acuity pediatric patients.
Qualifications
- Active, unrestricted RN/LVN license (PA).
- Current CPR certification.
- Experience with pediatric home health and high-acuity patients.
Responsibilities
- Provide skilled pediatric nursing 1:1 care in patients’ homes.
- Administer medications, treatments, and therapies.
- Support hospital-to-home transitions for high-acuity patients.
- Educate and support parents, families, and caregivers.
- Assist with discharge planning and care coordination.
- Train and support permanent field nurses during transitions.
- Provide first-shift and interim coverage as needed.
- Collaborate with Clinical Care Managers and interdisciplinary teams.
- Respond appropriately in emergencies and changes in patient condition.
