Advocate Health's Virtual Rehabilitation Program seeks a Speech-Language Pathologist to evaluate and treat communication, cognitive, voice, and swallowing disorders via telehealth. You will join an interdisciplinary team delivering patient-centered care to a broad community.

The role requires a Master's Degree in Speech-Language Pathology and a state license, with 40-hour work weeks and a full-time schedule. Remote collaboration across regions is expected.

Qualifications

• Requires a Master's in Speech-Language Pathology and licensure.
• Experience with telehealth and outpatient care preferred.
• EPIC documentation experience preferred.

Responsabilités

• Assess and diagnose communication, cognitive, voice, and swallowing disorders via telehealth.
• Develop and implement individualized treatment plans.
• Provide patient and family education and discharge planning.
• Collaborate with an interdisciplinary team to coordinate care.
• Document progress and outcomes in the medical record.
