Job summary

Advocate Health’s Virtual Rehabilitation Program seeks a Speech-Language Pathologist to provide evaluation and treatment for communication, cognitive, voice and swallowing disorders via telehealth. This full-time role supports an interdisciplinary team delivering patient-centered care across the community.

Requires a Master’s in Speech-Language Pathology, valid state licensure, and BLS. Preference for outpatient and telehealth experience; resides in Greater Charlotte NC area.

Qualifications

• Experience with outpatient healthcare setting and telehealth.
• EPIC documentation experience also preferred.
• Resides in the Greater Charlotte NC area.

Responsibilities

• Assesses and documents patient’s medical, mental and emotional needs at admission and ongoing; develops a plan of care.
• Acts as patient advocate; treats patients and families with compassion and implements plan of care in a safe and timely manner.
• Demonstrates clinical reasoning to conduct accurate assessments and evaluate therapy interventions.
• Identifies the normal course of illness in assigned patient population and intervenes when deviations occur.
• Communicates patient information and documents therapist’s actions and plan of care.
• Provides clinical leadership and mentoring.
