Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• Active LMSW/LSW required.
• 6 months to 1 year of job-related experience required (Graduate internship may be applied to the job-related experience at the discretion of the Director).

Responsibilities

• Manage complex biopsychosocial assessments and establish care plan goals.
• Attend patient care rounds and provide inpatient admission screening.
• Provide crisis intervention for interpersonal violence, psychiatric crisis, child abuse, and family disputes.
• Deliver therapeutic, goal-directed counseling.
• Coordinate with community services, including healthcare, legal, child welfare, and government programs.
• Coordinate safe and sustainable discharge with family involvement.
• Document social work notes in the patient medical record per department policy.
• Provide information on community resources based on assessed needs.
