Job summary

The Nemours Foundation seeks a Master’s Level Social Worker to provide clinical social work in a fast-paced healthcare setting, primarily covering evening, overnight, weekend, and holiday shifts.

You will perform psychosocial assessments, address safety concerns, provide crisis counseling, coordinate resources, and document in the EMR, collaborating with an interdisciplinary team.

Applicants should hold a CSWE-accredited MSW and Delaware LMSW/LCSW licensure with at least 6 months of experience.

Qualifications

• Master’s degree in social work from CSWE-accredited program.
• LMSW/LCSW licensure in Delaware required.
• Minimum 6 months social work experience.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families with complex concerns.
• Use advanced clinical judgment to identify risks and interventions.
• Assess safety concerns including abuse, self-harm, DV, substance use and SDOH.
• Manage high-volume caseloads with emergent patient needs.
• Provide crisis intervention, emotional support, counseling, and resource coordination.
• Collaborate with interdisciplinary team to develop patient-centered care plans.
• Facilitate referrals to community resources and behavioral health services.
• Maintain accurate documentation in the EMR.
• Demonstrate sound decision-making in fast-changing situations.
