Job summary

Nemours Children’s Hospital is seeking a dedicated social worker to conduct psychosocial assessments and support coping for patients and families within the inpatient setting.

The role includes crisis intervention, care coordination with multidisciplinary teams, linkage to community resources, and ensuring safe, timely discharge with family involvement while maintaining professional standards in documentation and collaboration.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Active LMSW/LSW required.
• 6 months to 1 year of related experience (graduate internship may apply)

Responsibilities

• Perform psychosocial assessments and support coping for patients and families.
• Provide crisis intervention for violence, psychiatric crises, child abuse, and family disputes.
• Coordinate with community services including healthcare, legal, child welfare, and government programs.
• Plan and facilitate safe, timely discharge with family involvement.
• Document social work notes in the medical record per department policy.
