Affirmed Home Care, Inc. is seeking a compassionate Pediatric Registered Nurse (RN) to provide skilled, one-on-one care for a 1-year-old with a tracheostomy, ventilator, and feeding tube at home in Hartford County, CT.

Per diem shifts include day, evening, overnight, and weekends. The ideal candidate holds an active CT RN license, current BLS, and at least 1 year of pediatric nursing experience, with strong preference for home-care pediatric exposure.

Qualifications

Active Connecticut RN license (CT RN) required.
Current BLS certification.
Minimum 1 year of pediatric nursing experience preferred.
Tracheostomy, ventilator, and feeding-tube experience strongly preferred.
Pediatric home care, hospital, or acute-care experience preferred.
Authorized to work in the United States.
Valid driver’s license preferred.
Physical exam within past 12 months.
TB screening or chest X-ray within past year, as applicable.

Responsibilities

Provide skilled, one-on-one pediatric nursing care at home for a 1-year-old with tracheostomy, ventilator, and feeding tube.
Collaborate with family and clinical team to develop and follow care plans.
Document care activities and changes in patient record.
Maintain infection control and patient safety standards in home setting.
