Job summary

Connecticut Children's, the state’s only health system dedicated entirely to children, is expanding its All Pointe HomeCare team with a Per Diem RN. This role involves delivering essential pediatric nursing care in patients’ homes across various CT locations, ensuring safe, compassionate, and high-quality care for children in a familiar setting.

Ideal candidates hold a Connecticut RN license and bring experience in pediatric or home-care settings.

Qualifications

• Current RN license in Connecticut.
• Experience in home care or pediatric settings preferred.
• Strong knowledge of medical terminology and diagnostic evaluation processes.
• Excellent communication skills to interact with children and their families.

Responsibilities

• Coordinate total patient care by conducting comprehensive health and psychosocial evaluation.
• Evaluate effectiveness of nursing service to the client and family.
• Consult with the attending physician concerning alterations to care plan; discuss with the appropriate supervisor to make changes as needed.
• Submit clinical notes, progress notes and other documentation outlining services rendered.
• Participate in case conferences; discuss with the appropriate supervisor potential concerns.
• Continually strive to improve nursing care skills by attending in-services and other educational opportunities.
