Qualifications

• Associates degree in nursing required.

Responsibilities

• Perform initial patient assessments and document findings.
• Develop and update care plans with patient and family input.
• Coordinate multidisciplinary care and discharge planning.
• Monitor patient progress and adjust care as needed.
