Job summary

Thom Child & Family Services in Westfield, MA seeks a Registered Nurse to deliver home- and community-based early intervention for infants and toddlers with developmental delays or risks. You will coach families, assess progress, and create individualized plans in partnership with caregivers.

The role requires MA RN licensure, strong documentation skills, and the ability to work independently or with a team to support child growth and family goals.

Qualifications

• Active MA RN license required and capable of home-based service delivery.
• Experience with early intervention or pediatric populations preferred.
• Comfort working in coaching model with families.

Responsibilities

• Participate in multidisciplinary assessments of developmental status.
• Assess family strengths, concerns, and progress in home/community settings.
• Provide child and family centered services using parent coaching.
• Coordinate services by scheduling and monitoring intervention plans.
• Partner with families to identify IFSP goals and strategies.
• Facilitate parent-child interactions to support development.
• Complete progress notes and reports using Thom system.
• Provide interventions for infants/toddlers 0–3 with delays or risks.
