Job summary

Children’s Healthcare of Atlanta is seeking a Registered Nurse — Case Management to coordinate clinically-based, evidence-driven patient care across inpatient and outpatient settings. Qualifications

• 3 years RN experience with a BSN
• OR a current Children’s RN with 5 years’ experience with an ASN; must obtain BSN within 2.5 years of hire date
• 3 years of broad clinical experience, predominantly in pediatric care

Responsibilities

• Completes initial screen of all patients on admission within 24 hours
• Develops and implements a robust transition care plan
• Coordinates care with clinical team to drive discharge planning
• Communicates with multidisciplinary team to facilitate care coordination
• Coordinates with home health agencies and third-party payors for discharge needs
• Facilitates care conferences and interdisciplinary rounds
• Performs outpatient and clinic care coordination as patients transition between settings
• Refer cases to risk management, peer review, or quality teams
