Job summary

Children’s Healthcare of Atlanta is seeking a Registered Nurse Case Manager to coordinate transition care for Behavioral Mental Health and Neuropsychiatry patients. You will screen admissions, develop care plans, and collaborate with clinical teams to ensure safe, high-quality discharge planning.

Ideal candidates have 3 years RN experience with BSN or ADN pathway, Georgia licensure, and strong communication and coordination skills to support families and multidisciplinary teams.

Qualifications

3 years RN experience with BSN or 5 years RN with ADN as alternative
Current Georgia RN licensure or multi-state license via eNLC
Experience in pediatric or behavioral/mental health care

Responsibilities

Complete initial screen within 24 hours of admission using trigger criteria
Develop and implement robust transition care plans with clinical team
Coordinate with medical staff, social workers, and discharge planning
Communicate with home health, payors, and community resources
Lead care conferences and rounds; monitor care during transition
Refer risk, peer review, or quality issues to appropriate personnel
Performs other responsibilities as required
