Qualifications

• RN with BSN required or ADN with plan to obtain BSN within 2.5 years
• Experience in pediatric care preferred
• Strong discharge planning and care coordination abilities

Responsibilities

• Complete initial screen within 24 hours of admission.
• Develop and implement robust transition care plans with the clinical team.
• Coordinate with medical staff, social workers, and others to drive care coordination.
• Communicate with all MDT members to facilitate care coordination for assigned workload.
• Liaise with home health agencies, third-party payors, and other resources as needed.
• Facilitate care conferences and rounds; monitor patient transitions between settings.
