Responsibilities

• Case management of sub-specialty pediatric patients for coordination of care.
• Utilize critical pathways and best practice guidelines for inpatient and outpatient care.
• Collaborate with patients, families, specialists, pediatrics, and multi-disciplinary teams for safe, cost-effective care.
• Organize, educate, and schedule pediatric sub-specialty procedures, appointments, nurse visits, etc.
• Coordinate patient care conferences and educational offerings.
• Primary contact for patients and families for triage, education, refills, and scope questions.
• Participate in quality improvement and data collection/analysis.
• Document in the patient’s medical record and support changes in care delivery.
