Job summary

Viva Pediatrics is seeking a Registered Nurse (RN) Case Manager to coordinate high-quality pediatric home health services for medically fragile children in the Austin area. This full-time role offers a Monday–Friday in-office schedule, collaborative team environment, and opportunities to mentor field nurses while ensuring continuity of care and compassionate, family-centered support.

Qualifications

• Texas RN license or multistate license valid in Texas.
• Pediatric or home health experience preferred.
• Experience with medically fragile pediatric patients highly desirable.
• Proficient in clinical documentation and EMR systems.

Responsibilities

• Coordinate individualized care plans for pediatric patients with complex medical needs.
• Oversee safe, effective, home health services.
• Collaborate with physicians, families, nurses, therapists and other healthcare professionals to support outcomes.
• Conduct patient assessments and monitor changes in medical status and care needs.
• Maintain accurate clinical documentation and care plans in accordance with regulations and policies.
• Coordinate patient admissions, recertifications and care plan updates.
• Provide clinical guidance to field nurses and caregivers.
• Monitor patient progress and ensure staffing continuity.
• Communicate with families and the interdisciplinary team regarding plan changes.
• Support quality assurance, compliance, and patient safety initiatives.

What You'll Do

• Manage and coordinate individualized care plans for pediatric patients with complex medical needs.
• Oversee the delivery of safe, effective, and compassionate home health services.
• Collaborate with physicians, families, nurses, therapists, and other healthcare professionals to support positive patient outcomes.
• Conduct patient assessments and monitor changes in medical status and care needs.
• Complete and maintain accurate clinical documentation, assessments, and care plans in accordance with applicable regulations and company policies.
• Coordinate patient admissions, recertifications, care plan updates, and ongoing case management activities.
• Provide clinical guidance, education, and support to field nurses and caregivers.
• Monitor patient progress and help ensure appropriate staffing and continuity of care.
• Communicate with families and the interdisciplinary team regarding changes in patient needs and treatment plans.
• Support quality assurance, compliance, and patient safety initiatives.

Qualifications

• Current, active Registered Nurse (RN) license in the state of Texas or a multistate license valid for practice in Texas.
• Previous nursing experience required; pediatric nursing and/or home health experience strongly preferred.
• Experience caring for medically fragile children with complex medical needs is highly desirable.
• Knowledge of pediatric conditions and treatments involving G-tubes, tracheostomies, ventilators, seizure disorders, and other complex care needs is a plus.
• Strong clinical assessment, communication, organizational, and problem-solving skills.
• Ability to work collaboratively with families, field clinicians, physicians, and office staff.
• Strong attention to detail and the ability to manage multiple patient cases and deadlines.
• Proficiency in clinical documentation and electronic medical record systems.
