Job summary

Shriners Children’s Hospital in Chicago, IL seeks an experienced RN Care Manager to assess, plan, implement and follow through for patients and families across the continuum of care.

The role requires a BSN and active RN license, with 3 years nursing experience. Preferred MSN and Case Management Certification; 1 year of care coordination and familiarity with InterQual criteria are highly regarded.

Qualifications

• BSN required and RN license in state of practice.
• 3 years nursing experience required.
• MSN and Case Management Certification preferred or to be achieved within 12 months of eligibility.
• 5+ years of clinical experience in Pediatrics, Burn care and/or Orthopedics also preferred.
• 1 year Care Coordination/Case Management experience.
• Experience with utilization review/management and InterQual criteria.

• Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team.
• Develops a safe and effective plan of care through early identification and assessment of patient needs and available resources.
• Collaborates with patients, families, caregivers, and the health care team locally and within SHC system to communicate, problem solve, and share accountability for outcomes.
• Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.
• Ensures prudent utilization of resources by evaluating the resources available to the patient and balancing cost and quality.
• Documents assessments, interventions, plans and evaluations per regulatory requirements and SHC policy.
