Responsibilities

• Performs comprehensive assessments on assigned patients with complete documentation.
• Communicates changes in patient condition promptly to the care team.
• Develops and updates the plan of care using nursing diagnoses and measurable goals.
• Identifies patient needs and prioritizes nursing diagnoses for care planning and referrals.
• Collaborates with patient, family and health care team for discharge planning and referrals.
• Evaluates patient responses to interventions and updates the care plan as needed.
