Job summary

Honor Health Network is seeking skilled and compassionate Registered Nurses to join our Connecticut home health team. This part‑time role offers the autonomy of field work, the support of an experienced clinical team, and the chance to build meaningful patient relationships.

This three-day-per-week position involves skilled nursing visits, comprehensive assessments, care plan updates, and education for patients and caregivers, all coordinated with the care team to ensure safe, high‑quality care

Qualifications

• Active Connecticut RN license (required)
• Preferred: Home health experience, including OASIS
• Accepted: Hospital, med-surg, telemetry, LTC, SNF, or rehab
• Strong clinical assessment skills
• Valid driver’s license, reliable vehicle, and auto insurance
• Excellent communication and documentation skills
