Job summary

Honor Health Network is seeking skilled and compassionate Registered Nurses to join our Connecticut home health team. This part-time role offers field autonomy, supported by an experienced clinical team, and the opportunity to build meaningful patient relationships with in-home visits.

The position is 3 days per week with competitive pay, orientation, and ongoing training as we expand statewide. You will deliver high-quality 1:1 patient care and collaborate closely with physicians and therapists.

Qualifications

• Active Connecticut RN license (required).
• Home health experience preferred, including OASIS.
• Hospital, med-surg, telemetry, LTC/SNF or rehab backgrounds may apply.

Responsibilities

• Provide skilled nursing visits and comprehensive assessments.
• Create, implement, and update individualized care plans.
• Deliver teaching, medication management, and wound care.
• Communicate with physicians, therapists, and the care team.
• Document all care accurately and on time.

Job Summary

Qualifications

• Accepted: Hospital, med-surg, telemetry, LTC, SNF, or rehab

• Strong clinical assessment skills

• Valid driver's license, reliable vehicle, and auto insurance

• Excellent communication and documentation skills
