Job summary

Qualifications

• Must be a licensed Registered Nurse.
• Experience in ambulatory care is preferred.

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process.
• Coordinate patient assessment and re-assessment to positively impact quality outcomes.
• Provide patient/family teaching and assess learning barriers.
• Respond to patients and families courteously, promptly, and professionally.
• Maintain privacy and confidentiality per HIPAA; document in Epic.
• Facilitate interdisciplinary collaboration and in-basket/pharmacy communications.
• Triage phone calls, manage messages, and schedule appointments by acuity.
• Document all communications and maintain patient confidentiality.
• Pursue ongoing learning; monitor quality dashboards; support PRIDE values.
