Job summary

Advocate Health is seeking a part-time Physical Therapist for the Pediatrics Acute Therapy unit at Advocate Christ Medical Center. The role involves evaluating patients, developing and implementing care plans, and collaborating with physicians and the care team to achieve optimal outcomes.

You will document progress and participate in discharge planning and family education. Candidates must have a Doctorate in Physical Therapy and be licensed in the relevant state; BLS is required.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation results and goals.
• Monitors and adjusts treatment plans based on patient response and provides status updates to physicians and teammates.
• Plans for discharge including home programs, patient/family teaching, and referrals or equipment as needed.
• Completes all required documentation including evaluations, progress notes, and discharge summaries.
