Job summary

Advocate Health Care is seeking a part-time Physical Therapist to interpret referrals, perform comprehensive evaluations, and develop individualized care plans. The role emphasizes collaboration with physicians and the care team to achieve optimal patient outcomes.

Responsibilities include monitoring progress, adjusting plans, discharge planning with home programs, and thorough documentation in line with department policies.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state of employment; BLS for Healthcare Providers within 90 days.
• No prior experience required; orientation to department policies.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluation results and goals.
• Monitors outcomes and adjusts treatment plans; communicates with physicians and team.
• Completes patient discharge with home programs and family teaching; coordinates with community services when needed.
• Documents care, billing, and scheduling; adheres to policies and accreditation standards.
