Job summary

Royal Care CHHA is seeking a dedicated Pediatric Physical Therapist to provide home-based care for children in Queens and Nassau County. You will partner with families to restore motion and help clients function at their personal best.

Requirements include a PT degree, valid license, and at least 1 year in home care (Medicare Part A). Bilingual skills, reliable transportation, and strong communication are pluses. Competitive pay, paid vacation, mileage reimbursement, and referral bonuses offered.

Qualifications

Proven work experience as a physical therapist.
Graduate degree or greater in physical therapy.
Valid license to practice physical therapy.
Any bilingual skills are a plus.
At least 1 year experience as PT in home care (Medicare Part A).
Reliable transportation is a must.
Strong communication, organization and time management skills.
Problem-solving and interpersonal skills.
Excellent computer skills.
Ability to work independently with minimal supervision.
Experience using HomeCare HomeBase (HCHB) is a plus but not required.

Responsibilities

Assess patients and create care plans in collaboration with physicians or as prescribed.
Document visits and track patient progress.
Educate caregivers and families on patient care.
Select appropriate care and services for each patient.
Support the company mission by delivering high-quality home care.
