Qualifications

• Master’s Degree in Physical Therapy required.
• Current state license and CPR.

Responsibilities

• Evaluate and conduct pediatric physical therapy in home health settings.
• Direct patient and caregiver in prescribed tasks to restore and enhance performance.
• Coordinate care with office staff and other disciplines.
• Maintain communication with patient and family about progress and home programs.
• Refer patients to appropriate services and document progress with physician.
