Job summary

KidsCare Home Health in Texas is seeking a licensed Physical Therapist to provide pediatric home-based therapy. You will evaluate children, design treatment plans, and coach families to support progress in daily activities within the home setting.

You will collaborate with a multidisciplinary team, maintain thorough documentation, and participate in ongoing education and quality improvement efforts. A strong commitment to compassionate, ethical care and work-life balance is valued at KidsCare.

Qualifications

• Master’s degree in Physical Therapy required.
• Current state-issued professional license.
• CPR certification.
• Pediatric clinical/home health experience preferred.

Responsibilities

• Evaluate, organize, and conduct medically prescribed physical therapy for pediatric patients in home health.
• Provide clinical treatment including screenings, evaluations and family/caregiver training and counseling.
• Coordinate patient care with office staff and other disciplines.
• Maintain communication with patient and family regarding progress and home programs.
• Refer patients to appropriate services as needed.
• Consult with physician on patient progress and maintain records.
• Maintain supplies and equipment.
• Participate in Quality Assurance and Performance Improvement (QAPI) program.
• Attend Continuing Education activities and expand scope of service.
