Job summary

You will design and implement individualized treatment plans and educate patients and families to promote optimal health and wellness. The role involves comprehensive initial evaluations, goal setting, collaboration with care teams, documentation of progress, and participation in quality improvement initiatives.

Qualifications

• Doctoral, Master’s, or Bachelor’s degree in physical therapy or a PT certificate is required.
• BLS certification from the American Heart Association is required.

Responsibilities

• Performs comprehensive initial evaluations to identify functional deficits and develops a treatment plan.
• Provides skilled interventions tailored to patient needs using appropriate techniques.
• Collaborates with patients to set short- and long-term goals.
• Communicates effectively with patients, families, and other health care providers while ensuring confidentiality.
• Manages patients to improve movement and function and prevent disability.
• Maintains accurate medical records including evaluation findings, plan of care, and progress.

Education

Doctoral Degree, Masters or Bachelor's degree from an accredited physical therapy program or physical therapy certificate

Basic Life Support (BLS) certification from the American Heart Association
