The role focuses on comprehensive assessment, care planning, and interdisciplinary collaboration for children with complex, life-threatening conditions.

The position emphasizes inpatient and continuum-of-care involvement across hospital and community teams, with weekend/evening call coverage and ongoing

Qualifications

• Experience caring for children with complex, chronic or life-threatening conditions.
• Ability to work within an interdisciplinary team and coordinate care across inpatient, outpatient and community settings.
• Licensed to practice as APRN or PA in Colorado by date of hire; current DEA in Colorado or able to obtain by hire date.

Responsibilities

• Conduct comprehensive assessments of pediatric palliative-care patients and families.
• Develop, implement, monitor and adjust palliative-care plans with input from the care team.
• Coordinate care across inpatient, outpatient, and community settings; participate in quality improvement and education.
