The role focuses on Advanced Practice Provider leadership in pediatric pall care, with duties spanning inpatient and continuum of care across multidisciplinary teams.

The position involves comprehensive patient/family assessments, care coordination, and clinical education while advancing research and quality initiatives.

Qualifications

• Prior clinical experience involving care of children with complex, chronic and/or life-threatening conditions, or end of life.

Responsibilities

• Conduct comprehensive assessments of children and families with palliative care needs.
• Formulate and monitor palliative care plans with input from the team.
• Assist families to understand their child’s condition and treatment options.
• Coordinate care across inpatient, outpatient and community settings.
• Engage in quality improvement and professional activities.
• Maintain licensure and certifications.
• Rotating weekend and evening call coverage.
