Job summary

Advocate Health is seeking an outpatient pediatric physical therapist at the Cleveland Reg Rehab campus. The role involves evaluating patients, designing and implementing treatment plans, and coordinating with physicians and families to support discharge planning.

A Bachelor’s degree in Physical Therapy, a state license, and BLS certification are required, with at least one year of licensed PT experience preferred.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Current physical therapy license in applicable state.
• One year of experience as a licensed Physical Therapist preferred.
• Basic Life Support certification required.

Responsibilities

• Evaluate patients and develop treatment plans.
• Collect and interpret data from medical records and patients.
• Set functional goals and provide evidence-based interventions.
• Document evaluations, progress notes, and discharge summaries.
• Coordinate patient care and discharge planning.
• Provide coverage at other departments as needed.
• Supervise PTAs and Rehab Techs as required.
