Job summary

Advocate Health is seeking a licensed physical therapist to join the outpatient rehabilitation team (Pediatrics: Miller). The role includes evaluation, treatment planning, and collaboration with a multidisciplinary team to promote patient-centered outcomes.

The position requires a DPT/B.S./M.S. or equivalent degree, current state licensure, and CPR (BLS) certification. Full-time hours, Monday through Friday with one 4-hour Saturday shift every 8 weeks, and comprehensive total rewards are offered.

Qualifications

• BS, MS, or DPT degree from accredited physical therapy school.
• Current licensure in the state of applicable state.
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Adheres to general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.
• Provides patient care assessment and treatment. Ensures patient evaluations and treatments are comprehensive, functional and according to the Standard of Practice. Establishes a treatment plan with realistic goals.
• Maintains accurate documentation. Completes written patient evaluations, progress notes, and discharge notes per policy. Documents patient education, charges and ICD-10 codes; maintains patient records in a systematic order.
• Ensures that documentation is legible.
• Meets department financial and productivity standards; adjusts schedules to meet patient/department needs.
• Participates in process management to optimize quality and efficiency of services; identifies opportunities and communicates progress barriers to supervisor.
• Promotes professional development and education of other health care workers; shares clinical expertise with students and other professionals.
