Qualifications

• BLS certification within 30 days (AHA).
• Licensed Occupational Therapist (OT) – IL IDFPR.
• Bachelor’s degree in Occupational Therapy.

Responsibilities

• Performs patient transfers with assistance as needed and uses safe body mechanics.
• Designs and implements an individualized plan of care with goals, treatment, outcomes, and discharge plan.
• Educates patients, families, caregivers, and staff using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations with reliable methods based on chief complaint, screenings, and history.
• Screens patients to determine need for further therapy services and uses support personnel appropriately.
• Monitors care by staff and provides feedback; documents evaluations and plans of care per policy.
• Completes annual education as required for HIP and DNV program requirements.
