• Interprets physician referrals and evaluates patient ADLs and functional abilities.
• Plans and implements individualized treatment based on evals and goals.
• Provides and fabricates assistive devices and splints.
• Monitors outcomes and adjusts treatment; shares updates with physicians.
• Identifies factors affecting physical and psychosocial comfort.
• Plans discharge, teaches home programs, and orders equipment.
• Collaborates with stakeholders to achieve optimum care.
• Completes all required.
