Qualifications

Master's Degree in Social Work from an accredited program.
Licensed Social Worker (LSW) is required.
Licensed Independent Social Worker (LISW) preferred within 1 year.

Responsibilities

Performs psychosocial assessment/intervention for patients and families.
Provides age-specific care to patients across all age groups.
Develops and documents comprehensive discharge plans aligned with family goals.
Links patients to community resources and UH network services.
Collaborates with the care team in a multidisciplinary approach.
Supervises social work interns (LISW eligibility required).
