Responsibilities

• Assess patients on admission and continually monitor status.
• Plan and coordinate care based on assessments and patient needs.
• Involve family/significant others in the plan of care.
• Initiate individualized teaching for discharge and positive outcomes.
• Collaborate with the multidisciplinary team to achieve patient goals.
• Initiate and verify physician orders and administer medications as policies.
• Maintain patient safety, confidentiality and professional conduct.
