Job summary

Community Care of West Virginia is seeking a Nurse Practitioner to provide ambulatory care under the supervision of the Director of Medical Operations. The role includes examining, diagnosing, and treating acute and chronic conditions, performing procedures, ordering labs, and managing patient discharge summaries.

The ideal candidate will hold NP certification, WV RN license, and prefer a master's degree with at least one year of experience in a clinical setting.

Qualifications

• Successful completion of an approved nurse practitioner program.
• Currently approved by the boards to perform medical acts as a NP.
• Must be licensed as a registered nurse in the state of WV.
• Master's degree preferred.
• Minimum of one year's experience is required.

Responsibilities

• See ambulatory patients for evaluation, diagnosis, and treatment of illnesses.
• Perform physical exams and collect patient history.
• Prescribe medications within scope of practice.
• Order and interpret labs; provide patient counseling and discharge planning.
• Assist in procedures and follow-up visits as needed.
• Maintain accurate patient records and support billing.
