Dayton Children's is seeking a Care Coordinator Social Worker to join the Partners for Kids program at the Tech Town location. Candidates must have a Master's in Social Work, Ohio LSW license, and BLS within 60 days of hire. Work schedule is full-time, 40 hours, Monday through Friday, 8:00 a.m.

Qualifications

• Master's Degree in Social Work required.
• Ohio LSW license required (Board of Counselor, Social Worker, and Marriage & Family Therapist).
• BLS certification required within 60 days of hire.

Responsibilities

• Assess key patient needs and address local and systemic issues affecting care.
• Navigate the health care system for high-need patients and supervise care coordination.
• Plan for patient and family education and monitor progress toward goals.
• Promote patient safety and quality of care across settings (inpatient, clinic, ER, home, community).
• Provide care coordination and advocacy in inpatient, outpatient, home, and telehealth contexts.
