Job summary

Qualifications

• Master’s degree in social work or related field required.
• Licensure: LSW, LPC preferred; LISW/LPCC acceptable.
• Active BLS certificate.
• 2 years clinical or medical social work experience required.
• Knowledge of Medicaid and regulatory standards preferred.

Responsibilities

• Outreach to healthcare providers to identify patients needing care coordination.
• Complete Health Risk Assessments and identify patient needs.
• Facilitate communication among the healthcare team and with families.
• Develop family-centered care plans and educate families.
• Support transitions across care settings and provide follow-up.
