Catholic Charities of the Archdiocese of Galveston-Houston is seeking a Medical Coordinator to coordinate and deliver medical services for unaccompanied children in shelters, schools, and group homes in the Houston-Galveston area.

You will work with a registered nurse to ensure timely exams, immunizations, screenings, and medical documentation, while maintaining HIPAA compliance and upholding program policies. This is a full-time, on-site role offering professional growth.

Formación

• Licensed practical or vocational nurse (LPN/LVN) or higher health-related qualification, OR RN with BSN.
• 1 year of public health or healthcare setting experience in care coordination or direct clinical care.
• Bilingual in Spanish and English. Valid driver's license.

Responsabilidades

• Ensure the safety and security of all children in care at all times.
• Provide quality care and supervision in accordance with ORR and state rules.
• Maintain ratio and close supervision through activities.
• Report health doubts or concerns to supervisor immediately.
• File and protect medical records confidentially.
• Communicate health information per policies to ensure quality services.
• Coordinate with Registered Nurse to implement care plans.
• Maintain documentation: biographical data, assessments, MARs, TARs, etc.
• Complete annual trainings for compliance and CAN certification.
• Assist in development or provision of medical services as approved.
