Compass Connections in Cameron facility, TX, is hiring skilled nurses to provide safe and compassionate care for children in emergency shelter and group home settings. The role requires licensure in the state, bilingual English/Spanish, and at least 1 year of healthcare experience with care coordination.

Qualifications

• Graduate of an accredited school of nursing with active license in the state.
• BLS required; CPR and PALS preferred.
• Must have immunizations or titers as specified.
• Experience in care coordination in healthcare settings.

Responsabilités

• Meet federal and state guidelines and program standards.
• Document properly in medical records and portal systems.
• Assess health conditions and administer vaccines, meds, and treatments as ordered.
• Coordinate healthcare appointments and transportation for patients.
• Maintain confidential PHI and follow privacy protocols.
• Communicate with Lead Medical Coordinator on policy issues.
• Provide crisis intervention and coordinate with case managers.
• Maintain safety and comply with safety protocols.
