Job summary

National Youth Advocate Program (NYAP) in Roselle, IL seeks a Medical Coordinator to arrange and coordinate medical, dental, mental health, and pharmacy services for children in care. You will document services, maintain confidential records, and support care teams to ensure timely, quality health outcomes.

You will collaborate with healthcare providers and foster families to schedule appointments, prepare documentation, and ensure compliance with confidentiality and program guidelines.

Qualifications

• Licensed Practical or Vocational Nurse (LPN/LVN) or higher health-related qualification (e.g., RN with a bachelor’s)
• Bilingual fluency in English and Spanish is required
• Minimum of 1 year in public health or healthcare setting with care coordination/health administration
• 1 year of experience in the child welfare field with children/adolescents in social services
• Must have a valid driver’s license and a safe vehicle
• Proficient use of computers and standard software

Responsibilities

• Arrange and coordinate medical, dental, mental health, and pharmacy services for children in care.
• Maintain confidential health records and ensure PHI protection.
• Coordinate appointments, intake/discharge, and care plans with care teams and providers.
• Prepare and submit documentation, TARs, and medical summaries; upload to portals.
• Support education for foster parents on medical policies and required duties.
• Ensure compliance with NYAP policies and health record confidentiality requirements.
• Collaborate with cross-functional teams to ensure appropriate medical care and approvals.
• Participate in ongoing trainings and contribute to quality improvement measures.
