Job summary

KidsCare Home Health in Dallas, TX is seeking an LPN/LVN Care Coordinator to support nursing teams, call in verbal orders, track signed orders, and coordinate daily operations for continuity of care. This role requires an LVN/LPN license, 2–5 years of pediatric nursing, and experience in home health, with the ability to manage scheduling changes and interactions with doctors and families.

Qualifications

• Reading/writing and public speaking in small groups; may be bilingual.
• Ability to perform basic math operations.
• Proficiency with Microsoft Office and basic database tasks.
• Handle multiple tasks and problem-solve with few variables.
• Knowledge of pediatric medications and interactions.

Responsibilities

• Monitor monthly and quarterly KPI performance.
• Call in nursing verbal orders promptly to maintain continuity of care.
• Assist with contacting doctors to verify paperwork and status of orders.
• Verify patient PCP and well-check information as needed.
• Update physician information in KanTime regularly.
• Process daily paperwork in KanTime.
• Maintain open communication with nursing teams, caregivers, and other departments.
• Assist with order entry and scheduling changes in the ticketing system.
• Follow safety policies and report unsafe activity to supervisor.
• Provide leadership by example and knowledge sharing.
• Participate in team efforts to achieve department and company goals.
• Perform other duties as assigned.
