Qualifications

• Current Pennsylvania LPN license in good standing
• BLS/CPR and First Aid certification required
• Must be at least 21 years old
• English reading/writing/speaking ability required

Responsibilities

• Deliver nursing care per resident care plans
• Administer medications and treatments safely
• Monitor residents and communicate concerns promptly
• Collaborate with physicians and other clinicians
• Contribute to admission evaluations and care planning
• Maintain accurate clinical documentation and records
• Provide shift handover reports for continuity
• Escalate needs to RN when required
• Participate in orientation and ongoing training
