Community Care, Inc. Home Health in Pittsburgh, PA is seeking a compassionate LPN to provide 1:1 pediatric private duty care for an 11-year-old boy with a trach/vent. The role emphasizes safety, family-centered care, and collaboration with a dedicated care team.

You'll perform ADLs, g-tube care, monitor vitals, and administer medications, delivering high-quality pediatric home health nursing with weekly pay and strong orientation.

Qualifications

• Current LPN (or RN) license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification.
• Reliable transportation and a valid driver's license.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Perform g-tube care/feedings.
• Provide trach/vent care.
• Monitor vitals.
• Administer medications safely and accurately.
• Communicate clearly with the care team and family.
• Deliver exceptional, family‑centered pediatric care.
