Job summary

The Staff Pad is partnering with a respected hospice provider in the DFW Metroplex to seek a compassionate Pediatric RN Case Manager who will deliver hospice care to children at home. This field-based role reports to the Managing Director - Hospice and coordinates all aspects of care with an interdisciplinary team.

The ideal candidate has a TX RN license and pediatric RN experience, with CPR certification and a valid driver's license.

Qualifications

• Graduate of an accredited Diploma, ADN, or BSN Nursing program.
• Current Texas RN license.
• Minimum one year of pediatric RN experience.
• Hospice, Critical Care, or Emergency Room experience preferred.
• Current CPR certification.

Responsibilities

• Conduct comprehensive pediatric home health assessments.
• Develop, implement, and document individualized plans of care.
• Coordinate with patients, families, physicians, and the care team.
• Provide clinical guidance to Hospice Aides and LVNs; assist with training.
• Complete pediatric admissions and schedule prioritization.
• Participate in on-call rotations for emergency patient support.
• Maintain documentation and ensure infection control and agency policy compliance.
• Support ongoing quality improvement and professional development.

The Staff Pad is proud to partner with a respected hospice provider in the DFW Metroplex, seeking a compassionate Pediatric RN Case Manager to provide exceptional hospice care to children and their families in the comfort of their homes.

Reporting to the Managing Director - Hospice, the Pediatric RN Case Manager, will provide skilled nursing care to pediatric hospice patients while coordinating all aspects of their care.

Working alongside an interdisciplinary team, physicians, and community partners, you’ll help ensure every child receives compassionate, dignified, and personalized care.

Schedule: Full-Time

What You’ll Do Key Responsibilities

• Conduct comprehensive physical, psychosocial, and environmental patient assessments
• Develop, implement, and document individualized plans of care
• Communicate effectively with patients, families, physicians, and the care team
• Provide clinical guidance to Hospice Aides and LVNs and assist with training as needed
• Complete pediatric admissions and prioritize patient scheduling
• Participate in assigned on-call rotations for emergency patient support
• Maintain accurate documentation and ensure compliance with infection control and agency policies
• Support ongoing quality improvement and professional development
• Perform other duties as assigned

What We Are Looking For Qualifications

• Graduate of an accredited Diploma, ADN, or BSN Nursing program
• Current Texas Registered Nurse (RN) license
• Minimum one (1) year of pediatric RN experience in a clinical setting required
• Hospice, Critical Care, or Emergency Room experience preferred
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• Current TB testing and Hepatitis consent/declination documentation
• Ability to pass a criminal background check

• Ability to stand, walk, lift, and safely transfer up to 50 lbs
